New Client Welcome
Welcome to the Duluth Animal Hospital family! In preparation for your appointment, we have a couple of items to help expedite your check-in and prepare us for your visit. We are so very excited to meet you and your pet family member!
Medical Records
Please provide us with the names of all previous veterinary facilities so that we can obtain medical records on your pet. We prefer to receive these records with at least 24 24-hour’ notice to allow time for the veterinarian to review all medical history and for us to update all reminders for vaccines and testing. If the records are in your possession already, please forward them to admin@duluthanimalhospital.com.
Due to privacy policies, we may not release information without the direct consent of the pet owner. If you must request records, please request all medical records, including lab results and doctor exam notes.
